Methylcobalamin, or Methyl B12, is a form of vitamin B12 naturally occurring in the body. Cyanocobalamin, on the other hand, is a synthetic B12 compound. Cyanocobalamin needs to be broken down in the body first to cobalamin and cyanide to be converted to one of the active forms of B12. Also, Cyanocobalamin is more excreted in urine compared to methylcobalamin, indicating that methylcobalamin is more bioavailable. In addition, scientists are concerned about the accumulation of cyanide that can occur when taking cyanocobalamin supplements. For these reasons, it has been proposed that cyanocobalamin stop being used as a B12 supplement now that more bioavailable, and potentially safer forms, such as methylcobalamin, are available.

Vitamin B12 exists in the human body in two primary forms: Methylcobalamin and Coenzyme B12, also known as Adenosylcobalamin. Both forms work as cofactors to two essential enzymes. Methylcobalamin serves as a cofactor for methionine synthase, an enzyme regulating homocysteine levels and S-adenosylmethionine (SAMe) levels. Coenzyme B12 functions as a cofactor to L-methylmalonyl-CoA mutase. This enzyme is required for hemoglobin synthesis and is involved in an essential biochemical reaction that metabolizes fat and protein. Together these enzymes are involved in various critical metabolic processes in the body, including neurological function, DNA and RNA synthesis, protein and lipid metabolism, red blood cell formation, and prevention of homocysteine (HCY) accumulation. HCY in excess levels has been identified as a risk factor for cardiovascular diseases, neurodegenerative diseases, and neural tube defects.
